What is BMI?

BMI stands for Body Mass Index. It is a simple numerical value calculated from a person's weight and height. The formula is:

BMI = Weight (kg) ÷ Height (m)²

For example, if you weigh 68 kg and your height is 1.70 metres, your BMI would be: 68 ÷ (1.70 × 1.70) = 68 ÷ 2.89 = 23.5. This value then falls into a specific weight category — in this case, "Overweight" by Indian standards or "Normal" by WHO standards.

BMI was devised by Belgian mathematician Adolphe Quetelet in the 1830s and is sometimes called the Quetelet Index. It gained widespread clinical use because it is easy to calculate, requires only two measurements, and correlates reasonably well with body fat percentage in most adults. However, BMI is a screening tool, not a diagnostic one — it does not directly measure body fat and cannot distinguish between fat mass, muscle mass, bone density, or fluid volume.

BMI Categories — Indian Standards vs WHO

India has adopted lower BMI cut-offs than the WHO because research has shown that Indians are more susceptible to metabolic diseases such as type 2 diabetes, cardiovascular disease, and hypertension at lower BMI values. This is partly due to differences in body composition — Indians tend to have higher body fat percentage and more visceral (abdominal) fat at the same BMI compared to Europeans.

The Indian guidelines, recommended by the Consensus Guidelines for Asian Indians (2009) and adopted by the National Institute of Nutrition (NIN), use a normal range of 18.5 to 22.9. This means that a person with a BMI of 24 is classified as overweight by Indian standards but normal by WHO standards. Given India's rising burden of lifestyle diseases, using the Indian cut-offs provides a more accurate health risk assessment for the Indian population.

Limitations of BMI

While BMI is a convenient and widely used metric, it has several important limitations that everyone should be aware of:

  • Does not distinguish muscle from fat: Athletes and bodybuilders often have a high BMI due to increased muscle mass, not excess fat. A muscular person may be classified as overweight or obese despite having very low body fat.
  • Does not account for age: BMI categories are the same for all adults, but body composition changes with age. Older adults tend to lose muscle mass and gain fat, so a "normal" BMI may mask underlying health risks.
  • Does not account for gender: Women naturally have more body fat than men at the same BMI. Using identical cut-offs for both sexes may underestimate risk in men and overestimate it in women.
  • Does not measure fat distribution: Where you carry fat matters. Abdominal (visceral) fat is more dangerous than fat stored in the hips and thighs. Two people with the same BMI can have very different health risks based on waist circumference.
  • Not suitable for children: For children and teenagers, BMI is age- and sex-specific, plotted on growth charts as BMI-for-age percentiles. Adult BMI categories should not be applied to anyone under 18.
